Transition Support Services

As Norfolk Public School strives to “…ensure that all students maximize their potential…” individualized support is available for students who are re-entering the traditional public school setting from alternative placements, corrections/detention, and/or the foster care system. Transition support services may involve a review of academic and discipline records, communication with the parent/guardian and student, identification of strengths and weaknesses, goal setting, recommendations for success, and the assignment of a mentor. A student-centered transition support plan may be developed and implemented at the student’s home school with support from an assigned central office administrator. Data will be regularly collected and reviewed and progress will be monitored so that the transition support plan will be amended as needed.
