Prior to participation in any NPS school-sponsored activity, each student-athlete must complete and submit the following to their school's Athletic Director on an annual basis - 

1. Sports Physical: dated by physician on 5/1/23 or later to be valid for the 2023-24 School Year 
2. Cardiac Arrest & Concussion Preparticipation Forms: reviewed and signed by parent/guardian and student-athlete
