Application for Tuition and Assessment Reimbursement
All full-time, contracted administrators and teachers are immediately eligible to request tuition reimbursement upon hire. Classified employees must be employed in a full-time position for at least two years to be eligible to request tuition reimbursement for a two-year college; classified employees must be employed in a full-time position for at least four years to be eligible to request tuition reimbursement for a four-year college or university.
Specific request procedures are outlined in the Application for Tuition & Assessment Reimbursement.

Employee Checkout (Form A-37)
Upon the retirement or separation of an employee from Norfolk Public Schools, the following checkout list must be completed prior to departure from the primary place of employment.

If you would like to change your name, upgrade your license, or add a new endorsement to your license, please complete this form.

Name or Address Change Request
All name change requests must be supported by the appropriate documentation.
Current employees are required to use Employee Self-Service to edit telephone and address information. Please visit https://norfolkpublicschoolsva.munisselfservice.com/ess/login.aspx. If you experience difficulty logging in to the system, please contact Help Desk at helpdesk@nps.k12.va.us.
Former employees should complete the Name or Address Change Request Form to edit telephone and address information.

Personal Leave Conversion Request
Employees who have at least two (2) unused personal leave days may request to have two (2) unused personal leave days converted to one (1) personal leave day to be added to next year's personal leave.

Request for Family and Medical Leave of Absence (FMLA)
This form is required from all employees requesting FMLA. This is in addition to the U.S. Department of Labor certification form. Links for request-specific forms appear below:

Resignation and Extended Leave of Absence (Form A-30)
Any employee requesting separation of employment or an extended leave of absence of up to one year must complete and submit this form.

Sick Leave Transfer Request
Any new employee wishing to transfer up to 100 sick days from his or her previous school division must complete and submit this form.

Verification of Employment - Current or Former Employees
Current or former employees who wish to have their employment and/or related information verified by the Department of Human Resources must complete, sign, and submit this form.

Verification of Experience - Incoming Employees
This form is required of all new employees and is used to verify previous work experience(s).
