Parents/guardians it is required that you submit documentation regarding your student's absence. This can be a certified physician note and/or an email given required information. Please, include the student's name, date of birth, date(s) of absence, and reason for absence. In addition, any student absent more than three days requires a certified physician note, parent written notes are not accepted. Reference
