Homebound Services
Homebound instruction is a process that should be navigated collaboratively between parents, schools, and the appropriate medical providers. Parents who wish to request homebound instruction for their student should request a conference with the school to determine the best recommendations to encourage academic success for their student. Homebound requires a licensed physician, psychiatrist, or a licensed clinical psychologist to certify the need for a student to be homebound.

3. School Recommendation (Pages 4 and 5)
The parent completes components one and two above. Once Parent and Medical forms are complete, please return them to your child’s school and the homebound liaison will complete the school’s portion no later than 48 hours after the application has been submitted by the parent.
