Part I: Cardiac Arrest & Concussion Preparticipation Forms

Prior to participation in any sponsored athletic activity, parents/guardians & student-athletes must review the NPS information about Sudden Cardiac Arrest, Concussion in Sport and Heat Related Illness annually- 

    1. Print & sign the document below (en Español)

    2. Submit the signed forms to your school's Athletic Director

Part II: Sportsmanship Acknowledgment Form

Prior to participation in any sponsored athletic activity, parents/guardians & student-athletes must review the NPS Sportsmanship Acknowledgment Form- 

    1. Print & sign the  document below (en Español)
