OptimaHealth Online Medical Enrollment and Benefit Changes for 2007-08You MUST have an Optima user name and password in order to enroll on-line. If you selected a user name and password last year and have forgotten/misplaced your user name, please contact Optima Health’s member services at (757) 552-7110 for retrieval. You will then be able to reset your password. If you are currently enrolled with Optima and have not created a user name and password in the past, please follow the link below to create your user name and password. If you are NOT currently enrolled in Optima coverage, please follow the appropriate link and you will be able to create a user name and password.
Note: You do not have to be enrolled in medical insurance through Optima to be able to take advantage of on-line enrollment.
On-line enrollment is only available for the following benefits:
- Medical Insurance with Optima Health
- Full time employees may choose from two Point of Service (POS) medical plans through Optima Health.
- Medical Insurance with ASI/Tricare Supplement
- Full time employees who are currently enrolled in Tricare may elect the supplement instead of coverage through Optima Health
- Dental Insurance/Dominion Dental
- Full time employees may choose from three dental plans through Dominion Dental. Plan offerings include one Dental Health Maintenance Organization (DHMO) plan and two Preferred Provider Organization (PPO) plans.
- Legal/Legal Resources
- The Pre-paid Legal Plan is available to employees to cover certain legal procedures. The employee pays a premium per paycheck for this service; however, does not pay an attorney when requesting a covered service.
- Long Term Disability/The Hartford*
- Long Term Disability provides income protection for employees who are out of work for more than 90 days due to their personal health status.
- Flexible Spending/WageWorks
- Medical Flexible Spending allows an employee to set aside pre-tax dollars for post-tax medical expenses. An employee may elect to have up to $4000 withheld annually. Dependent Care Flexible Spending allows an employee to set aside pre-tax dollars for post-tax dependent care expenses. An employee may elect to have up to $5000 withheld annually.
- Critical Illness Insurance/MetLife
- Policy pays a lump sum benefit if the employee experiences one of six covered conditions: cancer, heart attack, stroke, kidney failure, major organ transplant or coronary artery bypass graft. You may use this payment as you desire.
*If you have been employed with NPS for more than 30 days, an Evidence of Insurability form must be completed. The form is available on-line. Enrollment is subject to approval or disapproval from The Hartford.
Please note some of the important changes for the 2007/2008 school year:
On-line enrollment for coverage effective October 1, 2007 will be held from June 1, 2007 through June 29, 2007. Changes made outside of open enrollment will require a change of life status or qualifying event.
NPS will be offering a new insurance policy, Critical Illness Insurance. The Critical Illness policy provides a one-time lump sum payment for the diagnosis of several diseases. The intent of this policy is to assist with expenses associated with the illness. You can read more information about this product on-line or in your open enrollment packet.
As always, employees wishing enrollment in flexible spending MUST re-enroll/enroll each year.
